IMPORTANT NOTICE FOR COLLEGE OF HEALTH SCIENCES (CoHS) GRADUATES

This is to inform you that transcript collection is now conducted under the following
procedures
Part A: How to make request for a transcript?
To request for a transcript, submit a complete clearance form including bursar
clearance form and UDOSO clearance form. Together with the clearance forms, attach
your transcript fee receipt.
All the above mentioned documents have to be submitted at the dean's office of your
respective school before 1000 HRS.
NB: Make sure your passport size photo is already uploaded to your SR account. In case
of any inconvenience visit IT office.

Part B: Payments
Each transcript costs 15,000/= Tsh, you can get transcript with its certified copy for
20,000/= Tsh .

Part C: Collection
You can collect your transcript at the dean's office of your respective school only on
Monday, Tuesday and Thursday from 1230 HRS to 1400 HRS.
NB: Make sure you check your transcript for any informational error before leaving the
office.
